* ACHD will hold a Childhood Immunization Clinic at Geneva Public Library, 860 Sherman St, Geneva, OH, on the fourth Wednesday of the month — May 28 — from 12 pm to 2 pm.  No appointment is required.  Bring insurance cards and shot records if you have them. Physical Assessments Clinic

At our Physical Assessments Clinics we provide complete physicals for persons of any age.  Call the Ashtabula County Health Department at (440)576-6010, option #2, for more information and to schedule an appointment. 

Physical Assessments Clinics (also called a wellness clinic or well child clinic) will be held at ACHD on some Tuesdays and Thursdays of each month, by appointment only.

Monday, Tuesday, or Friday – walk-in or by appointment between  9:00 a.m.-3:30 p.m.

Please be sure we are in-network for your insurance carrier.  Free vaccine is available those who qualify.
