Ashtabula County Health Department will join with Ashtabula City HD to offer a Vaccination Clinic on April 3, 2025, from 3:30 – 5:30 pm at 4239 Lake Avenue, Ashtabula, OH. Measles-Mumps-Rubella(MMR), Varicella, DTaP, Polio and Flu vaccines for Adults and Children will be offered, and other vaccines will be available. Some vaccines are available at no cost to those who qualify. Bring insurance card and shot record, if available. Parent must accompany minor.

ACHD will hold a Vaccination Clinic on Monday, April 7th, at the Monroe Township Fire Department, 4095 Center Rd, Conneaut 44030. Adults and Children will be able to receive Measles-Mumps-Rubella (MMR), Diphtheria-Tetanus-Pertussis (DTP), Polio and Flu vaccines. The clinic will be open from 3:30 pm to 5:30 pm. Some vaccines are available at no cost to those who qualify. Bring insurance card and shot record, if available. Parent must accompany minor. At our Physical Assessments Clinics we provide complete physicals for persons of any age. Call the Ashtabula County Health Department at (440)576-6010, option #2, for more information and to schedule an appointment.
Physical Assessments Clinics (also called a wellness clinic or well child clinic) will be held at ACHD on some Tuesdays and Thursdays of each month, by appointment only.
